Supportive Housing Program

The Supportive Housing Program (SHP) will provide assistance to low income persons who are homeless, as defined by HUD, and disabled in a scattered-site tenant-based rental assistance program. This program is designed to link rental assistance with primary medical care. All referrals must come from the case manager.

Eligibility Section:
  • Case Manager assesses the need and sends Case Management Referral Form and budget.
  • Client must be homeless, as defined by HUD
  • Client must be willing to live in Missouri
  • Client must pay 30% of adjusted gross income toward rent and utilities
  • Client must abide by lease for at least 1 year
  • Client must be willing to get medical care at KC Free Health Clinic or TMC
  • Client must be willing to work with Peer Counselor at Kansas City Free Health Clinic or Truman Medical Center

    Assistance Section:

    An individual's or family's rent and utility assistance, if eligible, is based on Fair Market Rent as established by HUD for this region.
