Rental Assistance Due to Medical Crisis Program

The purpose of this program is to provide temporary rental assistance for individuals who experience a medical crisis due to a medical disability that forces the individual to stop working. Individuals must be in danger of becoming homeless due to the financial crisis created by their medical situation.

Eligibility Requirements & Program Guidelines

Participants will meet the following criteria:

  1. a. Be receiving critical care medical services through such programs as Medicaid Waiver, State Plan of Personal Care, or Hospice Care; OR
    b. be experiencing a medical crisis that has forced the individual to stop working (physician verification required); OR
    c. be seeking in-patient care for drug or mental health treatment.

  2. Have maintained regular, steady employment within a 45-day period prior to the medical crisis and previously had sufficient income to meet expenses.
  3. a. Have zero income; b. Have a financial burden created by out-of-pocket medical costs or extended hospitalization. Assistance will be based upon monthly income, not previous annual income.
  4. Not be receiving any other on-going rental assistance and have no other means to pay rent/mortgage or utilities.
  5. Provide copies of lease, proof of ownership, and/or utility bills.
  6. Be compliant with medical leave policy established by employer, if applicable.
  7. Apply for Section 8 within 30 days of first payment.
  8. Develop a plan for continued care within 30 days of first payment, as applicable (e.g. rental payment plan, stabilization of financial situation, verification of completed treatment plan, post treatment after care plan, etc.).
  9. Participants must be re-certified on a monthly basis by their Ryan White Case Manager.

Assistance

An individual who meets the above criteria would be eligible for:

  1. Rental/mortgage and utility assistance not to exceed fair market value based on household composition.
  2. Period of eligibility would be for up to 3-months.
